TLDR: TMI story about a client's bowel obstruction. Guy was sh*tting out his mouth and needed a hospital stay. DON'T let impactions go on for weeks!
OMG, I can't believe that woman put up with a bowel obstruction for so long! Bowel obstruction (from constipation or foreign bodies) is a medical emergency and can lead to perforations, sepsis, and death.
At my old job (group home for clients with Severe and Persistent Mental Illness) we had a client with a bowel obstruction who didn't tell anybody because he didn't want to go to the hospital (a fear I fully understood as he'd had traumatic experiences at the state hospital). We knew he wasn't feeling well yet couldn't figure out what was going on. We tried. He'd completely deny our concerns when asked if he was having a medical issue. The rules for our type of homes prevented staff from being physically intrusive… a very good thing, as the rule was made to prevent abuse. If we'd been able to check him physically, by touch or by sight, we would have noticed the bloating he'd been hiding under his clothes. Still, allowing staff to force clients to strip or let staff feel their body could be traumatic for clients, no matter the intentions AND would make it easy for staff with bad intentions to get away with all kinds of abuse. So not wanting to cross those boundaries, we watched and waited.
Eventually we started finding sh*t in trash cans around the house. We couldn't figure out why someone was crapping in trash cans, until someone saw the client vomiting in one of the cans. Technically, no one was crapping in trash cans.
However… The poor guy was so backed up his body was sending partially digested food out the only way it could 😬. The horror when we realized he had an obstruction AND had it so long that sh*t was coming up through his mouth… It was an immediate ambulance call for the guy, followed by an unfortunately lengthy hospital stay.
AMAZINGLY he didn't have a perforation. Anywhere. He still needed time in the hospital to recover after the impaction was removed. All us staff felt horrible for not catching it sooner and definitely wanted to prevent it from occurring again. When the client returned we had a long chat with him and his doctor, and worked out a plan to prevent another impaction (and another hospital stay): he'd report bowel movements to staff so we could track them. After a couple days without a BM we'd remind him about the laxatives he'd been prescribed for constipation. He'd ask for one and report back in about an hour, or whenever he had a BM.
It worked wonderfully: the client had no embarrassment around telling staff about his bowel movements and was quite thrilled when we expressed how much we really wanted to help him stay out of the hospital! He was fully on board with the plan, and it was still in place when I moved out of state several years later. He never had another obstruction/impaction.
We weren't able to find out how long he had the obstruction. The doctors at the hospital guessed it had been over a month. They were amazed that he avoided perforation because it usually happens much earlier.
If you've made it this far, I'm sorry for the rambling story. If anything, please know YOU SHOULDN'T WAIT weeks for medical help if you're constipated!
Get thee to an ER at the first sign of an impaction – if it's from constipation, or because something else got stuck up there. Waiting can literally turn an easy visit into something requiring surgery/into a life threatening situation. Don't let embarrassment or fear kill you- the ER docs have seen it all before. Also, they'll lose their jobs if they tell anyone outside the hospital unless you've given them legal permission to do so.

see capitalism is just so evil and backwards and antithetical to the human condition. we naturally want to help each other and work together but then some producer artificially creates boundaries like team vs team and the cash prize which disincentivizes them from working together. one team is clearly struggling with one of their members fainting because they're struggling to find food and they reach out for help from another team, and they naturally want to help but the artificial boundary has been set up to pit them against each other (the money and the teams they're forced into)
